Ausgangslage

Der Schlaganfall ist eine Erkrankung des höheren Lebensalters und ist mit rund 59.000 Verstorbenen in 2015 (ICD-10 I61, I63, I64, I69) die dritthäufigste Todesursache in Deutschland [1]. Abschätzungen zur Häufigkeit und der Krankheitslast (burden of disease) innerhalb der Bevölkerung sind deshalb notwendige Grundlagen für die Berechnung des zukünftigen Versorgungsbedarfs und bestimmen die gesundheitspolitische Planung. Als ab 1994 in Deutschland die ersten Stroke Units (SU) entstanden, wurde von den Kostenträgern die Frage nach der Gesamtzahl der zu versorgenden Schlaganfallpatienten aufgeworfen. Leider gab es zu diesem Zeitpunkt keine belastbaren Zahlen für die Bedarfsermittlung von SU. Dies war die Geburtsstunde des Erlanger Schlaganfall Registers (ESPro).

Mit einer Anschubfinanzierung des Bayer. Staatsministeriums für Gesundheit nahm am 01. April 1994 das ESPro seine Arbeit auf. Das ESPro ist mit 10.500 Fällen nicht nur das größte bevölkerungs-basierte Schlaganfall Register, sondern mit einer kontinuierlichen Datenerhebung über einen Zeitraum von derzeit 29 Jahren (1994-2023) eines der ältesten Register weltweit. So konnten in 2017 insgesamt noch fünf Teilnehmer aus dem Jahr 1994 aufgesucht und befragt werden. Wegen seines Alleinstellungsmerkmals wird das ESPro seit 2000 vom Bundesministerium für Gesundheit (BMG) im Rahmen der Gesundheitsberichterstattung des Bundes gefördert.

Methode

Das ESPro erhebt fortlaufend, ohne Unterbrechung seit 29 Jahren (1994-2023) Daten zu Epidemiologie, Krankheitsverlauf, Versorgung und Gesundheitsökonomie der Volkskrankheit Schlaganfall und Demenz. Das ESPro ist als ein regionales, bevölkerungs-basiertes Register konzipiert, in das möglichst alle diagnostizierten Schlaganfälle, das heißt sowohl hospitalisierte als auch nicht-hospitalisierte Patienten, unabhängig von der Altersgruppe und Schweregrad der Ausprägung, eingeschlossen werden. Die erfassten Patienten innerhalb der Stadt Erlangen (Einwohnerzahl zum 31.12.2020: 112.385) werden einem engmaschigen Verlaufsmonitoring unterzogen und in Abständen von 3, 12 Monaten und danach jährlich bis zum Lebensende weiterverfolgt. Diese langfristige Nachbeobachtung ermöglicht eine umfassende Versorgungsforschung zur Volkskrankheit Schlaganfall. Direkte Informationen zum Verlauf der Erkrankung, deren Behandlung und Versorgung können aus den Registerdaten abgelesen und in Präventionsmedizin, Therapie und Versorgungsplanung eingebracht werden.

Epidemiologie des Schlaganfalls

Das ESPro konnte erstmalig für Deutschland belastbare, bevölkerungsbasierte Zahlen zur Inzidenz und Sterblichkeit des Schlaganfalls vorlegen [2] und darauf aufbauend zeitliche Trends für den Zeitraum 1994-2010 berichten [3-5]. Die aus dem Register generierten Informationen gehen in die aktuelle Gesundheitsberichterstattung ein und sind Bestandteil des jüngsten Berichtes ’Gesundheit in Deutschland’ des Robert Koch-Instituts von 2015 [6].

Risikofaktoren des Schlaganfalls und Komplikationen

Vom ersten Tag der Erkrankung werden im ESPro die Risikofaktoren erfasst, überprüft und dokumentiert. Im Rahmen der Weiterverfolgung der Patienten werden Änderungen im Risikoprofil, das Auftreten von Schlaganfallrezidiven sowie die Spätfolgen, wie z. B. kognitive Einschränkungen und Anfallsleiden untersucht [7-15].

Versorgungsbedarf des Schlaganfalls über die Sektorengrenzen hinweg

Das 29-jährige Follow-up mit Datenerhebung über die Sektorengrenzen zwischen stationärer und ambulanter Versorgung hinweg sind die Alleinstellungsmerkmale des ESPro. Diese Darstellung der gesamten Versorgungskette der Akutbehandlung, Rehabilitation, Pflege und der hausärztlichen Betreuung ermöglicht nicht nur die Identifikation von Bereichen der Über-, Unter- und Fehlversorgung, sondern auch eine Bewertung der Langzeitversorgung [16-20].

Behandlungskosten des Schlaganfalls in Deutschland

Die detaillierte Erfassung auf Patientenebene machte die Darstellung des Ressourcenverbrauchs im Rahmen der Langzeitversorgung möglich [21]. Das ESPro hat im Rahmen seiner Mitarbeit im Kompetenznetz Schlaganfall [22] erstmalig die lebenslangen medizinischen Behandlungskosten (direkte Kosten) des Schlaganfalls ermittelt [23]. Zudem war es möglich, die Gesamtkosten der zukünftigen Versorgung von Schlaganfallpatienten in Deutschland hochzurechnen. Da die Patientinnen und Patienten im ESPro im Rahmen einer epidemiologischen Studie erfasst wurden, können die erhobenen Daten als repräsentativ angesehen werden.

Ausblick

Mit seiner Langzeiterhebung von epidemiologischen, medizinischen und Versorgungsdaten stellt das ESPro ein Zukunftsmodel für eine umfassende und nachhaltige Versorgungsforschung der Volkskrankheit Schlaganfall dar. Mit seinen Informationen unterstützt das ESPro Entscheider im Gesundheitswesen und leistet seinen Beitrag zur evidenzbasierten Gesundheitsversorgung in Deutschland.
